1. ?-May-99: arrival at LANL, visually inspected
2. ?-May-99: at LANL, glue output cable to MCM
3. ?-May-99: sent to Pace to install surface mount components
4. ?-May-99: visual inspection, no problems reported.
5. 14-Jul-99: at LANL, failed MCM QA test two bad blocks of 32 channels (7th and 8th) -- AMU/ADC problem? .
6. 6-Aug-99: repeat MCM QA, failed -- same problems, two bad blocks of 32 channels .
7. 6-Aug-99: probe surface mount components, no problems seen
8. 11-Aug-99: sent to Pace to have 4 surface mount caps replaced
9. 24-Aug-99: repeat MCM QA, failed -- same problems, two bad blocks of 32 channels .
