111 C to C 7/15 to 8/6 "3rd block of 32 channels dead to cal-injection, later 5th block showed anomaly". 3rd block worked (but low gain). 5th block dead (always 3FF). 3rd block recovered (maybe ramp setup was improved?). stayed in clean tent.

109 C to F 8/6 to 8/20 2nd preamp dead. 2nd preamp OK, but 5 of 8 AMU/ADCs didn't work. At least 4 of these AMU/ADCs worked on August 6th -- the other associated with previously dead preamp. short in surface mount cap for that preamp found. Short fixed at PACE. Other surface mount components changed.
112 A to C- 7/8 to 8/23 grade A. In the laser station, 4th group of AMU/ADC channels all overflow, 24 other channels dead. To check: Do these channels respond to bench-cal? attached to Si+cable assembly. Surface mount components changed, shipped to CA twice.
113 F to F- 7/19 to 8/6 only first 32 channels worked none of the channels worked. As far as I know, this MCM remained in the clean tent without being touched.
116 A to C 6/? to 8/19 This MCM was not put through the "official" QA testing. It was one of the first 3 MCMs we received and was extensively tested. We did not find problems. Si+MCM assembly tested on MCM test stand. 5th block of channels didn't work (AMU/ADC problem). The 4th and 6th groups failed intermittantly, but usually worked. attached to a Si+cable assembly, surface mount components changed, shipped to CA twice
120 A to F 8/? to 8/18 "worked" sometime during the week of August 9-13. However, it was never put through the "official" QA tests. Failed completely. It remained in the clean tent. We believe this was damaged in handling.
