1. Fill out this form (#1861)
   • Josh also has copies of this form.
   • All you need is a signature of the host and visitor.
   • Anybody is allowed to be a host.
2. Make 3 or 4 copies:
   • one to be with the computer,
   • one for the host to keep,
   • If the visitor is a foreign national, send a copy to: ISEC, MS P133
   • and one to give to Josh
3. Have an authorized person hook you up with a purple cable (Josh, Shawn, Mike?, Hubert?)
