History of MCM 112

1. ?-May-99: arrival at LANL, visually inspected
2. ?-May-99: at LANL, glue output cable to MCM
3. ?-May-99: sent to Pace to install surface mount components
4. ?-May-99: visual inspection, no problems reported.
5. 7-Jul-99: at LANL, passed MCM QA test .
6. 9-Jul-99: attached to Si detector via Kapton cable
7. 9-Jul-99: send to Pace for wirebonding and encapsulation
8. 20-Jul-99: send to Pace to replace 4 surface-mount components
9. 5-Aug-99: probe surface mount components, no problems seen
10. 23-Aug-99: repeat QA tests: one block of 32 channels plus 20 other channels dead in tests on laser station.
11. 26-Aug-99: "gently spread 5 min epoxy over encapsulate"
